Passport Program Support
If you or a family member receive Ontario Passport Funding, Qualicare is eligible for Passport Program reimbursement. Through the Passport Program, you can arrange for support from Qualicare which you will then be directly reimbursed by the Ministry of Community and Social Services Passport Office.
Flexible and Customized
Our program to support your needs is unique in its flexibility. We can provide as much or as little as you need. We can provide frequent care or infrequent care. You can call us up as needed, when needed. There are no contracts, no commitments and no obligations.
Our focus is to provide one-on-one out-bound services allowing us to remain flexible to your needs. We can provide extended support for vacations, weekends or respite care. As well, we can provide one shift as needed, when needed. Unlike cellphone companies, we have no contracts so you have no long-term obligations. (We do require 24hr cancellation notice)
Personal 360 Approach
We support your goal to participate and to be active in your community. We can support outings, shopping trips, or social events. Attending school, training for employment, or volunteering is also part of our support. Passport is designed to encourage your personal development and we are there to support you reaching your potential. Our goal is to help smooth out the road ahead.
When you call Qualicare, you get all the benefits of Qualicare Service and everything we do is supervised by a nurse manager under our 360 approach. Our support and supervision is your safety net. If you need we can expand our support to become advocates or navigators because this is the core of what we do. It all depends on your needs, your goals, and your plans.
To support families and current caregivers, Qualicare’s service includes as needed respite support. It can be as little as a 2hr shift to go shopping or it can be an extended period. Call us in advance with as much notice as possible to ensure coverage when you need it. Our support for your support helps you recharge so you can re-engage.

We are proud of each member of our care team. They are trained, professional and reliable. All care team members are employees (not subcontractors or independents) We screen all staff, carry comprehensive Liability Insurance, WSIB and have strict policies to protect you and your family. As a company, we are also certified and credentialed with federal, provincial and local organizations, HCAI, FSCO, VAC/VIP, and the Grand River Hospital
While our local office is in Kitchener. Our care team extends support throughout Waterloo, Cambridge, Stratford, Woodstock, Brantford, Hamilton, Simcoe, Dunnville, and surrounding areas
About Ontario Passport Program
Passport is an Ontario program that helps adults with a developmental disability be involved in their community and live as independently as possible. Run by the Developmental Services of Ontario (DSO), individuals must contact their local DSO office to apply.
Applications may be made by parents, family, or legal guardians of individuals

Passport Program Management Assistance
We are experienced with managing budgets, planning care and helping with administration. Self-directed care may feel complicated at the start but once you get going, it will feel natural. Budgeting and planning can be confusing so just ask and we can help.
The program is primarily a reimbursement program and our invoices provide supporting details to ensure the process is smooth and hassle-free. If needed, we can help organize direct transfer billing so there is no financial burden to you.
